For many older adults in need of long term care services, home is their first choice for care. VNS CHOICE, a comprehensive managed long-term care program, enables over 3,600 serving frail older adults in New York City to satisfy that preference.VNS CHOICE was established in 1998 as an innovative program of integrated care. Under the program model, care managers, or 'Nurse Consultants', lead an interdisciplinary team that also includes social workers, rehabilitation therapists, nurse practitioners and nutritionists. These clinical specialists collaborate closely with program member/patients, caregivers, community physicians, and other providers to achieve a common goal: helping member/patients remain independent at home and in their communities for as long as is safely possible.We are seeking talent for the opportunity described below.
Responsible for assisting in the development and implementation of strategies designed to increase the efficiency and effectiveness of the enrollment process. This includes education, marketing, facilitating the enrollment workflow at the regional level and administration of the enrollment process. Works under general supervision.Licensure: Current license and registration to practice as a Registered Professional Nurse in New York State required.Education: Bachelors Degree in Nursing from an approved program accredited by the National League of Nursing or Bachelors Degree in a health or human service field and 2 years experience in Long Term Care environment or a combination of education, experience and/or training required.Experience: Minimum three years experience in home care required. Minimum of one-year supervisory experience preferred. Knowledge of enrollment and delivery of care processes within a long-term care program, Medicare and Medicaid standards, and nursing needs of the elderly, chronically ill or disabled populations required. Proficient with personal computers including MS Office applications required.
