VNSNY CHOICE Community Care is a managed long term care program for frail older adults who wish to remain in their own homes to receive long term care services for as long as possible, but require ongoing assistance due to chronic illnesses and functional decline. The members of the program's highly collaborative care management teams include registered nurses and nurse practitioners, rehabilitation therapists, licensed social workers, and nutritionists. These professionals coordinate and/or provide home-, community- and facility-based long-term care and health-related services. We currently service more than 3,600 members in all five boroughs of New York City.We are seeking talent for the opportunity described below.
Responsible for providing physical therapy and overseeing rehabilitation therapy (physical, occupational, speech and language) services to patients in collaboration with the health care team consistent with VNSNY CHOICE policies. Acts as subject matter expert in support of managed care efforts in VNSNY CHOICE. Works under moderate supervision.
Licensure: License and current registration to practice as a Physical Therapist required. Valid driver's license may be required, as determined by operational/regional needs.Education: Bachelors Degree, Masters Degree or entry level Doctorate Degree in Physical Therapy from a program approved by the New York State Department of Education required.Experience: Minimum of one year of home care and/or acute care experience required. Geriatric experience preferred. Bilingual skills may be required, as determined by operational needs.
