Home care nursing is one of the fastest-growing areas of the nursing profession. Keeping patients safe and as healthy as possible in the comfort of their homes demands creativity, collaboration, and active listening. The Home Care RN provides one-on-one skilled nursing care for patients through professional knowledge and observation, interviewing, and physical examination. Ideal candidates will possess the ability and desire to work both autonomously in the field, as well as collaboratively with a multidisciplinary team.
As a Home Care RN, you will provide skilled one-on-one nursing care to maximize your patient’s physical, cognitive, social, and behavioral well-being. This role requires a solid foundation of both clinical and assessment skills coupled with strong critical thinking, communication, and interpersonal skills. Home care nursing delivers holistic, patient-centered care allowing patients to achieve a higher level of independence at home and in the community.

"As nurses, the skilled interventions we’re trained to do are the part of the job we can control. We can’t regulate people’s behavior, though. One of the biggest issues is getting patients to fully understand the part they need to play in their own care. Knowing how important it is to encourage such “health literacy” inspires me to be patient, compassionate—and persistent. Our VNSNY training teaches cultural sensitivity, which is helpful, but at the end of the day nurses also need to be clever psychologists to understand patients’ obstacles to compliance and self-care. How you talk to patients is just as important as what you say. Even as we’re educating, which is a huge part of the job, we need to be motivational as well. "

Looking for a Registered Nurse, RN to coordinate and provide high quality multidisciplinary health services for patients in the assigned region consistent with Agency philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision.
Licensure: License and current registration to practice as a registered professional nurse in New York State required. Valid driver's license or NYS Non-Driver photo ID card, may be required as determined by operational/regional needs.
Education: Degree in Nursing from an approved program required.
Experience for CHHA: Minimum of one year of experience in a medical/surgical environment preferred. Bilingual skills may be required, as determined by operational needs.
