The Home Care RN provides one-on-one skilled nursing care for patients through professional knowledge and observation, interviewing, and physical examination. Ideal candidates will possess the ability and desire to work both autonomously in the field, as well as collaboratively with a multidisciplinary team.
As a Home Care RN, you will provide skilled one-on-one nursing care to maximize your patient’s physical, cognitive, social, and behavioral well-being. This role requires a solid foundation of both clinical and assessment skills coupled with strong critical thinking, communication, and interpersonal skills. Home care nursing delivers holistic, patient-centered care allowing patients to achieve a higher level of independence at home and in the community.

Full-time staff earn up to 31 days of PTO (over a 12-month period)
Up to 18 credits per calendar year (or $12,000 per calendar year)
Provides high quality nursing care to meet the needs of the patient and manages/coordinates the delivery of cost effective multidisciplinary health care services for a caseload of patients consistent with VNSNY philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision.
Licensure: License and current registration to practice as a registered professional nurse in New York State required. Valid driver's license or NYS Non-Driver photo ID card, may be required as determined by operational/regional needs .NYS DL required
Education: Degree in Nursing from an approved program required.
Experience for CHHA: Minimum of one year of experience in a medical/surgical environment preferred. Bilingual skills may be required, as determined by operational needs.
