Work schedule: Monday - Friday, 9:00am - 5:00pm.
Engages and/or assesses geriatric individuals potentially in need of in-home mental health services with the goal of preventing psychiatric and, or alcohol or substance use related hospitalizations or long term care placement. This includes providing a substance use assessment and/or social services information and referral, Works under general supervision.
Education: Master's Degree in Social Work, Counseling, or related degree required.
Experience: Minimum one year of social work experience in a mental health or social service setting required. Bilingual in English / Spanish preferred.
