Mental Health Nurse Practitioner

Overview

A Mental Health Nurse Practitioner role is a comprehensive community support and treatment program for Severe & Persistent Mental Illness adults who have not responded to traditional modes of treatment. The program is a long-term program responsible for total outpatient psychiatric care. One must demonstrate history of multiple hospitalizations (3 in the past 2 years).


Responsibilities
  • The Per Diem Mental Health Nurse Practitioner is responsible for providing mental health assessment and treatment to patients with psychiatric symptoms, cognitive deficits, and/or behavioral problems. Provides advanced nursing care in collaborative practice with AMH psychiatrist
  • Works under general supervision
  • Conducts psychosocial assessments of patients and develops treatment recommendations.  Determines whether further evaluation by collaborating psychiatrist is warranted
  • Provides any of the following services to patients:  advocacy, psycho-education, psychopharmacology, cognitive/behavioral interventions, monitoring of mental status, emotional support, brief psychotherapy, family based interventions
  • Provides support and education to VNSNY staff and caregivers to enable them to better care for patients with psychiatric, cognitive and/or behavioral problems.
  • Consults with collaborating psychiatrist as necessary to develop psychiatric evaluation and treatment plan
  • Consults with referrer on assessment, recommendations, and update on progress of treatment
  • Assists the health care provider in implementing the plan of care
  • Follows a treatment plan, revising and updating as necessary; makes appropriate referrals for additional mental health services; provides crisis intervention as required
  • Participates in interdisciplinary team meetings, case conferences and clinical supervision; attends in-service education programs
  • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient

Qualifications
  • License and current registration to practice as a Registered Professional Nurse in New York State required
  • License as a Psychiatric Nurse Practitioner in the State of New York required
  • Valid drivers license may be required, as determined by operational/regional needs
  • Masters degree in psychiatric nursing required
  • Bilingual skills may be required, as determined by operational needs required
  • Minimum of one year of experience in a medical/surgical/psychiatric environment preferred CA2020
