Responsibilities and Qualifications
Conducts face to face comprehensive assessments on all new potential enrollees, as well as current members, to determine service needs and continued eligibility based on the member's needs. Ensures adherence to departmental, regulatory (i.e. federal, state, and local) and contracted MLTC requirements and data integrity standards. Works under general supervision.
Licensure: Current registration to practice as a Registered Nurse (RN) in New York State is required.
Education: Bachelor's or Master's degree in Nursing from an approved nursing program is preferred.
Experience: Minimum of two (2) years home care, hospice, or other community based nursing care experience is required. At least two (2) years experience completing UAS-NY Assessments is required. Ability to travel to a designated geographic area for in-person clinical assessment activities is required. Excellent organizational and time management skills, interpersonal skills, verbal and written communication skills required. Demonstrated strong relationship management and successful conflict management skills required. Working knowledge of Microsoft Excel, Power-Point, and Word required. Knowledge of Medicaid and/or Medicare regulations and services is required.#JT
